1. The final course evaluation aims to obtain students’ opinions toward the courses and teaching and improve communication between teachers and students. We appreciate you filling out the questionnaires.

1. Students who have completed the questionnaires for each course registered in this semester will be given the following qualifications:
   1. You will be given priority to check your semester grades from week 18 of this semester.
   2. You will be able to participate in a prize draw. The prizes include iPads, bonus money, and other prizes. Detailed information on the drawing process will be announced afterward.

2. Instruction
   1. Available time: from December 9, 2024 to January 11, 2025. Website: https://ceq.nkust.edu.tw/Home/StdIndex
   2. Please log in with your student ID and password (as the school affairs platform password). If you encounter any login problems, please get in touch with Computer and Network Center in NKUST (ext.13134).
   3. The user guide for the system is as the attached file.

3. Notice

1. The questionnaire is taken anonymously. Please feel free to answer.
2. Please complete each questionnaire within a period of time and submit it. After you submit the questionnaire, you cannot revise or delete the responses.
3. The organizer retains the right to replace the original prize with other products of equal value for the prize draw. The prize winners cannot request replacements or exchange the prize for cash. According to the regulations, international students must pay 20% tax on the winning prize.
4. The contents will be deleted if the responses contain abusive, vulgar words or personal attacks.
