如果有讓您感動的地方，請給老師一份鼓勵；如果有需要改進的地方，也請不吝分享。您的意見，是學校進步的關鍵，更是讓學習環境變好的最佳參與方式。

備註：

一、   凡於本學期教學意見調查施作期間填寫完所有課程應施測科目問卷之同學，獎勵如下：

(一)  提早查詢成績-自第十八週(114年1月6日)起，即可提早查詢老師已上傳課程之當學期成績。

(二)  獲得抽獎資格-有機會抽到iPad、一萬元獎金等多項好禮。填寫完全部課程問卷，才能參加抽獎喔！(公開抽獎日期及方式另行公告)

二、   填答方式：

(一)  填答期間：113年12月9日(星期一)至114年1月23日(星期四)。

(二)  登入路徑：本校首頁/在學學生/我有話要說/教學意見調查(網址： https://ceq.nkust.edu.tw/Home/StdIndex)，請輸入帳號(學號)、密碼(同校務行政系統密碼)，若無法成功登入可洽電算中心(分機13134)。

(三)  檢附操作手冊(如附件一)。

三、   注意事項：

(一)  本問卷採不記名填答，且老師打成績期間，無法看到同學填寫的內容，請同學安心作答。

(二)  每份問卷有一定的填答時間，請於時間內勾選完成並按下送出。送出後問卷狀態為「已完成」，無法受理作廢、註銷等申請。

(三)  獎項以實物與獎金為主，主辦單位有權以其他等值商品替代原贈品，獲贈者不得要求更換替代商品或將實物折換現金。獲獎者為本國生依規定須納入所得，外籍生依規定須繳納稅額。

(四)  填寫內容若有無理謾罵、粗鄙言詞或涉及人身攻擊，將逕予刪除。

1. The final course evaluation aims to obtain students’ opinions toward the courses and teaching and improve communication between teachers and students. We appreciate you filling out the questionnaires.
2. Students who have completed the questionnaires for each course registered in this semester will be given the following qualifications:
   1. You will be given priority to check your semester grades from week 18 of this semester.
   2. You will be able to participate in a prize draw. The prizes include iPads, bonus money, and other prizes. Detailed information on the drawing process will be announced afterward.
3. Instruction
   1. Available time: from December 9, 2024 to January 23, 2025. Website: https://ceq.nkust.edu.tw/Home/StdIndex
   2. Please log in with your student ID and password (as the school affairs platform password). If you encounter any login problems, please get in touch with Computer and Network Center in NKUST (ext.13134).
   3. The user guide for the system is as the attached file.
4. Notice

1. The questionnaire is taken anonymously. Please feel free to answer.
2. Please complete each questionnaire within a period of time and submit it. After you submit the questionnaire, you cannot revise or delete the responses.
3. The organizer retains the right to replace the original prize with other products of equal value for the prize draw. The prize winners cannot request replacements or exchange the prize for cash. According to the regulations, international students must pay 20% tax on the winning prize.
4. The contents will be deleted if the responses contain abusive, vulgar words or personal attacks.
